Please fully complete the form providing complete details about you, the person making the request and the individual who needs new ELEVATE user account credentials.
The sections marked with a star * must be completed, even if you are both the person making the application, and the proposed new user account holder.
If the new user account holder is required to order for multiple locations please indicate this in the comments section.
Most often, due to security considerations, an ELEVATE user account must be associated with an email address issued by your employer or the primary account holder.
The use of a personal email address is permitted in some instances, however we may need to obtain further information from either you, your head office or primary account holder proir to issuing new credentials. In these circumstances we will contact you to advise of the next steps.
